“Instant Permanent Teeth”™

Dr. Meltzer’s technique called “Instant Permanent Teeth”™ combines immediate placement with immediate loading. In some cases this approach can be combined with minimally invasive computer-navigated surgery. Dr. Meltzer has presented this new approach around the world. The approach has been presented before the American Academy of Periodontology and the American Academy of Oral and Maxillofacial Surgeons. Instant Permanent Teeth™ is a new buzzword in dentistry. Instant Permanent Teeth™ means replacing a single tooth, multiple teeth or a full set of teeth by inserting an implant or implants and on the same day attaching a temporary crown or a fixed bridge to the implant(s). The temporary crown or bridge is replaced by a more durable final prosthesis at a later date to maximize aesthetics and function.

Instant Permanent Teeth™ is a revolutionary concept providing patients with fully functioning non-removable teeth on dental implants in one single procedure that lasts about an hour. This technology was recently developed by Dr. Meltzer and others in conjunction with BIOMET 3i and allows for collaboration between both the restorative doctor and the surgeon. This merging of knowledge and experience achieves precise implant placement without scalpels and stitches. In addition, the fabrication of a set of non-removable teeth is completed prior to the surgery. The computer-guided implant surgery is done without requiring any flap reflection. This benefits the patient because there is less post-operative discomfort, less swelling and less bruising. Often patients can resume their normal activities the next day. This CT scan allows for the generation of a 3-D model of the jawbone, which can then be used in virtual reality software to plan the implant placement without the presence of the patient.

The results are more accurate implant placement and less chair time for the patient at the surgery office. Dr. Meltzer has done computer-guided dentistry for over ten years and is the first surgeon in the greater Philadelphia area to utilize this exciting new approach. He has been involved in the development and design of this new protocol as well as the alpha and beta testing phases. The design of the prosthesis was a result of a collaboration of Dr. Meltzer, Dr. Harold Baumgarten and Al Nelson CDT with BIOMET 3i. Our patient’s welfare is our number-one concern. Understanding Your Treatment Options and Paths

There are several treatment pathways to place and restore a dental implant. The best path for your treatment will be discussed and selected after your consultation.

Placement Options

  • Classic Placement – Classic placement involves the placement of an implant into a healed site (4-6 months or more after tooth loss).
  • Immediate Placement – Immediate placement involves the placement of a dental implant at the time of tooth extraction.
  • Delayed Placement – Delayed placement involves the placement of the implant following tooth extraction. This procedure usually involves bone and/or soft tissue augmentation. The delay could be as short as 8 weeks to as long as 9-12 months.
  • Implant Loading Options (How soon the tooth is placed on the implant)
  • Classic Loading – The implant is allowed to heal in a resting state for 3-6 months to allow the implant to integrate biologically with your jaw. Then the tooth or teeth are attached.
  • Early Loading – The implant is allowed to heal in a resting state for about 8 weeks. Accelerated integration with your jaw is achieved through the addition of a more biologically active surface to the implant. Dr. Meltzer has pioneered the use of Micro and Nano-implant surfaces for accelerated Bone Bonding™.
  • Immediate Loading – Implant loading is the placement of a temporary crown on the implant at the time of implant placement. There are two sub-headings for immediate loading:
    • Immediate Occlusal Loading – The immediately placed crown is in contact with the opposing teeth.
    • Immediate Non-Occlusal Loading – A crown is immediately placed at the time of implant placement, but is kept out of contact with the opposing teeth.

The scanning appliance is created from an existing denture or new wax-up to visualize the soft tissue and desired tooth positions.

A CT scan of the patient is taken. Data from the scan is converted into our planning software.
