During your patient’s diagnostic and treatment-planning phase as well as during and at the completion of your patient’s treatment, we will be in direct communication with you so follow-up treatment for restoring and protecting your patient’s dentition can be planned.

After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information.
